Mental Health Outreach
FCS acknowledges that mental health outreach services play a crucial role in providing support and care to individuals dealing with mental health challenges. Mental outreach services are designed to bridge the gap between mental health care and the individuals who need it most. These services aim to provide mental health support in a variety of settings outside of conventional health care facilities. By doing so, they make mental health resources more accessible, reduce the stigma associated with seeking help, and offer early intervention to prevent more severe mental health issues.
FCS recognises that there is shortage of community support available for most people affected by mental illness such as support at home, help with seeking employment, debt advice and access to independent advocacy which can exacerbate mental health problems and leave people not knowing where to turn. FCS offers continuing outreach support to working age and elderly people with mental health needs in their own homes to meet their health and social care needs such as encouraging them to observe personal hygiene, cleanliness, budgeting, accessing the community and participating in structured activities.
FCS mental health outreach services are offered based on care-planned person-centred needs which are reviewed regularly or as required by the service user or their next of kin. Services users in need of outreach services are risk assessed prior to a package of care being agreed. The amount of time required and payments for outreach services vary depending on individuals needs. Most service users in need of mental health outreach services are initially subjected to a Care Act Assessment (CAA) by their respective local social services who refer them to FCS Ltd.
