Understanding Your Personalized Care Plan with Us

Your choice of Flexible Care Solutions (FCS) shall be followed by the completion of a comprehensive Person-Centred Care Plan with you or your next of kin (family) to ensure all your support needs are captured accurately with consent. Your Person-Centred Care Plan shall be reviewed regularly to include your changing support needs or as may be required by you or your next of kin/family. The Care Plan shall identify your specific support needs and offer explanation on how they shall be met. We shall also identify the most suited care staff for you at this stage. We shall arrange for you to meet your carer/s to ensure that your needs are met, including any additional training requirements.
